Emergency Preparedness
In an emergency, it is easy to "forget" even the most well-known information. That is why it is crucial for you to have an up-to-date Emergency Medical Information Form (PDF) for rescuers. The Elm Grove Emergency Medical Technicians encourage everyone to complete the information in this form and have the form available in an emergency.
