2. Village of Elm Grove 2026 Liquor License Application

   You are applying for the July 1, 2026 to June 30, 2027 Liquor License. The application and forms from the DOR must be completed in order to be approved. There is a space below for Dance Hall Information if you need to apply - no separate application is needed. Licenses will be EMAILED TO THE APPLICANTS once approved. No licenses will be sent in the mail. The license must be placed in a visible area.

5. Please fill out the entire form before submitting. Any incomplete applications will receive an email denial with reason. Note: if there are any delinquent invoices due to the Village, the County, or a wholesaler you may not be approved until those invoices are paid in full. Complete payment should be mailed to the Clerk's office at 13600 Juneau Blvd, Elm Grove WI 53122.

    There will be a $25 fee associated with any Dance Hall License Application.

    By submitting this document, you acknowledge the following: I understand I must pay the license fee in full if I wish to have a liquor license issued to my business. I understand I can have no outstanding debt with the Village of Elm Grove and possibly Waukesha County. I understand the agent information is sent for a criminal background check, a legal notice for the license is placed in the paper, and the Board of Trustees must approve the license application before the Clerk can issue me my license. If I hold a reserve license, I understand my license is not eligible for a premises-to-premises transfer. I understand I cannot sell my retail alcohol license to another person or entity, that they must apply through the Village and I either relinquish/surrender my license within the year or I do not apply for a renewal of the license at the end of the license period.

66. Upon issuance by the Clerk, licenses will be emailed to the email address listed earlier in this form. Paper licenses will not be mailed except upon written request. Licenses must be printed and displayed so they are visible from the point of sale.
